| 설명 | 자연치아와 그와 연관된 조직이 결손되었을 때, 인공적으로 대치하는 보철물. 넓은 의미로는 인공치를 갖는 보철물을 뜻하는데, 주로 국부의치(partial denture)와 총의치(complete denture)를 포함하여 사용한다. 기록에 따르면, BC 500~400년경 이미 현재의 의치와 방법상 큰 차가 없는 것이 있었다고 한다. 즉, 고대의 의치는 금속선이나 띠고리로 상아를 비롯하여 소나 하마의 치아를 깍은 것을 잇거나 납땜질을 한 모양인데, 현재의 국부의치나 가공의치(bridge)등의 방법과 유사하다고 한다. 총의치는 17~18세기경, 특히 근세 치과학의 아버지라고 하는 프랑스의 P. 포샤르(1678~1761)는 현대의 것에 가까운 총의치를 처음으로 고안하였다. 현재 사용되고 있는 의치의 종류는 다음과 같다. ① 가공의치 : 하나 또는 둘 이상의 자연치아가 결손되어 있을 때 결손된 부분에 가공치를 대입하는 데, 가공치와 인접한 자연치아에는 지대치로 하여금 관을 씌워 금관과 가공치가 연결되어 구강내에 영구 정착되는 보철물이다. 그 형태와 기능, 제작법과 적응 등은 다양하다. ② 국부의치 : 하나 또는 그 이상의 자연치아가 결손되었을 때, 즉 치아 자체가 결손된 상태는 아니며 치아와 연관된 지지조직이 결손되었을 때 지탱해 주는 치과보철물로, 가철식 또는 고정식이 모두 포함된다. 부분저인 치아상실이나 조직의 결손을 인공적으로 대치하는 보철물이다. ③ 총의치 : 자연치아가 하나도 없는 환자에게 인공적인 방법과 수단으로 여러 가지 재료를 사용하여 제작하는 의치를 말한다. 실제에 있어서 치과보철의 최종적인 보철물이라 볼 수 있다. 환자는 총의치를 장착함으로써 음식물을 씹거나 발음기능을 발휘하고, 외관상 자연스럽고, 연조직에 해가 없어야 한다. |

| 설명 | 표준 기초대사량과 비교했을 때 개체 기초대사량이 보이는 편차를 표시하는 지수. 기초대사량은 생명을 유지하는데 필요한 최소한의 에너지 대사량이며, 성과 연령이 동일한 건강인의 기초대사량은 체표면적에 비례한다. 이것을 체표면적의 법칙이라고 하며, 1882년 독일의 대사생리학자 M. 브루너에 의해 제창되었다. 체표면적은 신장과 체중에 의해 산출된다. 따라서 성-연령-신장-체중을 알면 표준기초량 Y가 산출되고 실제의 기초대사량 X는 산소소비량과 이산화탄소 발생량에서 산출된다. 그리고 X와 Y의 차이를 Y로 나눈 값(%)을 기초대사율이라고 한다. 경험적으로 얻은 BMR의 간단한 측정법도 있으며 다음과 같은 식으로 구한다. 이 식에서 맥폭이란 최고혈압과 최저혈압의 차를 말한다. BMR(%)=0.75(1분 맥박수 + 0.74×맥폭)－72. BMR이 10% 이내이면 정상범위, +10% 이상이면 기초대사항진, －10% 이하이면 기초대사 저하라고 한다. 기초대사는 공복시(식후 10시간 경과)에 안정된 상태에서 단위 시간당 이용되는 에너지의 양, 즉, 한 시간당, 몸 표면의 1m2당 또는 몸무게 1kg 당 칼로리로 나타낸다. 이것은 개인에 따라 다르며 장기간에는 거의 변화가 없지만 질병 등으로 인해 변동된다. 실측한 기초대사를 표준치와 비교함으로써 질병의 진단 등에 응용할 수 있다 BMR=(실측치-표준치)/표준치 ×100(%)로 나타낸다. |

| 설명 | 음식-운동-정신감동 등 체온에 영향을 줄 만한 여러 조건을 피하여 몸과 마음이 안정되었을 때 잰 체온, 보통 아침에 눈을 뜬 직후에 재며, 여성의 경우 월경 주기에 따라 달라지므로 수태 조절이나 자궁, 난소의 병 진단에 응용한다. 6~8시간의 안정된 수면을 취한 후 아침 일찍 깨어나, 잠자리에서 나오기 전에 체온계를 입에 물어서 측정한다. 미묘한 체온의 차이를 측정하는 것이므로 여성체온계를 사용하는 것이 좋다. 이 여성체온계의 수은구부를 혀의 아랫면에 끼우고서 입을 가볍게 다물고 약 5분 동안 있다가 측정하면 된다. 정상적인 기초체온은 복잡한 성주기의 변화를 보이기 때문에 월경주기에 관한 각종 진단을 비롯하여 임신-유산-이상임신의 진단과 수태조절 등에 응용하는 등 여러 가지 효과적인 자료가 된다. |
