영문 Crohn's disease 한글 크론병
설명   
  만성적이고 재발을 잘하는 창자의 염증을 특징으로 하는 병. 장의 벽은 안쪽에서부터 점막, 점막하조직, 근육층, 장막의 4개의 층으로 이루어져 있는데, 크론병은 이 모든 층의 염증을 동반한다. 장의 모든 부분에서 생길 수가 있지만 주로 막창자와 연결되는 큰창자의 말단부에 가장 많이 생긴다. 창자의 전층의 염증으로 인해서 장의 폐쇄나 괴양을 만들며 종종 천공된다.
영문 ulcerative colitis 한글 궤양대장염
설명   
  잘룩창자(colon)의 만성, 재발성 궤양으로 염증이 주로 점막 및 점막 하부에 일어나는 원인불명의 대장염이다. 호발부위는 구불창자 및 곧창자이며 임상적으로는 경련성의 복통과 곧창자출혈, 혈액, 고름 및 점액을 포함한 설사가 특징적이다. 진단은 임상상과 대변검사, 그밖에 곧창자구불창자보개로 하며, 치료는 Sulfasalazine, 스테로이드, 수술 등이다. 합병증으로는 치질, 고름집, 천공, 암전환 등이 있다.

설명   
  큰창자에 생긴 염증을 말한다. 만성적이고 재발을 잘하는 대장의 염증을 특징으로 하는 병. 이중 궤양성대장염(ulcerative colitis)은 점막과 점막하조직에만 염증이 생기고 이 이하의 조직인 근육층과 장막층에는 염증이 없는 것이 특징이다. 대개 항문과 곧창자에 호발한다. 증상으로는 쥐어짜는 듯한 배아픔과 항문출혈, 대변속에 고름이나 이상한 점액의 배출 등이 있다. 이것이 오래 지속이 될 경우에는 대장암으로 이행될 수도 있다.

    크론이 1932년 처음으로 회장 말단의 장염을 국한적 회장염으로 기재한 것. 회장 말단염. 소장, 대장에서도 볼 수 있으며 회장 말단부의 거의 30cm에 자주 발생하고 점막에서 점막 하층에 미치는 염증성 변화로 육아 거대세포를 볼 수 있다. 원인은 장내 세균, 알러지 등의 연관된 것으로 보인다.

    세균 감염이나 장 내용물의 병적 발효 등으로 인한 대장의 염증. 폭음, 폭식이나 부적당한 식품, 음료수가 원인이 된다. 증상은 물 같은 설사가 특징인데, 하루 10회에서 수십 회에 달하며, 점액이나 불소화 음식물의 찌꺼기나 때로는 피가 섞이는 일도 있다. 그리고 대개의 경우에는 복통이 따른다. 복통은 산통이 일어나는 경우가 많다. 병의 원인에 따라서는 발열도 있고, 메스꺼움, 구토가 일어나기도 한다. 설사가 심하면 어린이나 노인은 탈수 상태가 되어, 경련, 기면 등 위독한 증세를 내기도 한다. 치료는 복부를 따뜻하게 하고, 식사는 유동식으로 한다. 탈수 상태가 있으면 포도당 액, 액 등을 점적 주사한다. 약제로서는 목탄 가루, 수탄 가루, 백도 흙 등의 흡착 수렴제와 술파제 또는 항생 물질을 투여한다. 복통에는 아트로핀계의 약제가 유효하다.

    1. 대장과 직장에 발생하는 만성적이고 우연적인 염증성 질병. 다양한 양의 혈액과 점액, 고름이 섞인 수양성 설사가 특징적인 소견이다. 2. 결장 궤양, 즉 결장의 만성, 재발성 궤양화로서 주로 점막 및 점막 하부에 일어나며 원인 불명이다. 임상적으로는 경련성의 복통과 직장 출혈, 혈액, 고름 및 점액 등에 아주 작은 분립이 섞인 설사가 특징이다. 병발증에는 치질, 농양, 누, 결장 천공 가용종 및 암 등이 있다. 3. 대장의 하부를 중심으로 광범위하게 궤양이 발생하는 질환. 궤양은 결핵이나 매독 등의 경우처럼 특이한 조직상을 보이지 않으므로, 비특이성 궤양성 염증이라고도 한다. 조직학적으로는 궤양성 변화와 함께 증식성 변화가 있는 것이 특징이다. 서양에서는 전체 인구의 10 % 정도가 이 병에 걸리며, 동양인에게는 적게 발생하나 식생활의 서구화에 따라 증가하는 경향에 있다. 원인으로는 감염설, 알레르기설, 교원병설, 정신 신경설 등 여러 가지를 들고 있으나 결정적인 것은 없다. 증세는 급성기에는 고열, 설사, 점혈변 등이 있고, 식욕 부진까지 겹쳐 몸이 쇠약해지기 때문에 적리로 취급되는 경우가 있다. 만성기가 되면 열도 내리고 변도 좋아지나 1년 이내에 재발하는 확률이 높고, 급성기 증세를 나타낸다. 이러한 급성 악화를 반복할 때마다 병변은 확대되어 심하면 궤양이 결장 전체로 퍼진다. 일반적으로 주장법 X선 검사와 직장경으로 진단할 수 있으며, 세균 검사에 의하여 세균성 질환을 제외하는 일도 필요하게 된다. 치료는 내과적으로 이루어지며, 경구 또는 주장 방식으로 항생 물질이나 술파제와 함께 부신피질 호르몬이 유효하며, 또 고단백, 고칼로리 식사를 하고 안정을 취하는 것도 중요하다. 병변이 심할 때, 재발을 반복할 때, 출혈이나 천공 등을 일으켰을 때는 결장의 절개 수술을 한다.

Crohn's disease <disease, gastroenterology> An inflammatory disease of the gastrointestinal tract that seems to have both genetic and environmental causes, not well understood.
The peak incidence of onset of this disease is between 15 and 25 years of age. Crohn's also occurs in later years between the ages of 55 and 60.
Common symptoms include recurrent abdominal pains, fever, nausea, vomiting, weight loss and diarrhoea which is occasionally bloody. Complications include gastrointestinal bleeding, fistulas and anal fissures.
Treatment includes anti-inflammatory drugs and corticosteroids. Surgery is successful in a select few.

Abdominal pain, diarrhoea, vomiting, fever, and weight loss can be symptoms. Crohn's disease can be associated with reddish tender skin nodules, and inflammation of the joints, spine, eyes, and liver. Diagnosis is by barium enema, barium X-ray of the small bowel, and colonoscopy. Treatment includes medications for inflammation, immune suppression, antibiotics, orsurgery.
