| 설명 | 위, 소장이나 대장에서 영양분을 흡수하기위한 모세혈관조직은 모두 간으로 연결이 된다. 즉 소화기에서 흡수한 영양분이 가득한 피는 모두 간으로 연결되는데 이것을 문맥계(portal system)라고 한다. 어떤 원인으로 인해서 문맥계의 압력이 높아지게 되는 것을 문맥고혈압(portal hypertension)이라 말한다. 문맥압의 상승이 있는 경우에는 문맥계와 연결이 되어 있는 모든 부분의 정맥의 압력이 높아지고 정맥의 순환이 정지된 상태가 된다. 식도의 정맥도 문맥계와 연결이 있어서 만약 문맥압의 항진이 있는 경우에 역시 이곳의 정맥의 혈액이 순환이 되지 않고 정체되어 있어서 많은 혈액이 식도의 정맥에 고이게 되어서 정맥의 확장이 일어난다. 이렇게 확장되어 있는 정맥을 정맥자루(varix)라고 부른다. 이 식도의 정맥자루는 자극에 의해서 파열이 될 수가 있고, 만약 파열이 되어 적절한 조치가 취해지지 않는 경우에는 출혈로 사망하기도 한다. 문맥고혈압의 원인으로는 간경화 등의 간질환, 문맥을 기계적으로 막는 질환 등을 들 수 있다. 이중에서 가장 많은 것은 간경화증에 의한 경우이다. |

| 설명 | 산성 위액의 작용에 의해서 식도, 위 또는 샘창자 점막이 파괴되어 결손부가 생기는 것. 즉 위벽이 허는 것인데 단 그 원인이 산성위액에 의하였을 때 한하여 소화성 궤양이라 한다. 소화궤양은 위산에 의해서 발생한 궤양을 말한다. 소화궤양의 발생메커니즘은 기본으로 위-샘창자의 보호메커니즘과 손상요인의 불균형으로 발생한다. 소화궤양은 대개 긴지름 0.6cm 이상인 경우가 많고, 절반이상은 긴지름이 2cm 이상이다. 전통적으로 소화궤양은 둥근 모양에 경계가 분명하며 궤양의 벽은 반듯하다. 궤양의 가장자리는 악성 궤양과 달리 두꺼워져 있지 않다. |

| 설명 | 압박으로 인해서 국소피부의 혈액순환이 감소되고 그것으로 인해서 피부조직이 파괴되는 것을 이르는 말이다. 즉 장기간 피부가 압박받아서 압박을 받은 부위의 혈관이 눌려서 혈류가 감소하고, 그 부분에 공급되는 혈류가 적어서 그 부분이 섞어서 피부의 조직이 없어지는 것을 욕창이라고 한다. 욕창의 호발 부위는 주로 압박을 받기가 쉬운 피부 부위로서 주로 뼈가 튀어나와 있어서 그 뼈로 인해서 쉽게 압력을 받기 쉬운 부위로 무릎, 발뒤꿈치, 엉덩이의 뒷면(엉덩이 뒷면에 뼈가 나온 곳) 등이 있다. 일단 발생한 욕창은 그 상처의 깊이에 따라 5단계로 분류한다. 제1단계는 피부의 압력에 의해서 피부에 적색 발적이나 피부가 단단해지는 정도의 변화를 말하고, 제2단계는 피부의 상층부위에 국한된 표층성 궤양이며, 제3단계는 피부 깊숙이 존재하는 피하지방조직까지 욕창이 침범하였으나 피하 지방층아래에 있는 근육층은 보존된 상태이며, 제4단계는 근육층까지 조직의 결손이 있는 경우이며, 제5단계는 광범위한 욕창이 관절이나 장기가 들어있는 몸속의 공간, 즉 체강까지 침범한 경우이다. |
