| 설명 | 풍선을 이용한 압력으로 구조의 내강을 넓히는 심장병 시술법. 협착이 있는 부위에 특수 제작된 풍선을 삽입한 후 고압의 공기를 일시적으로 주입하여 확장시키는 방법으로 수술자국을 남기지 않으며 입원기간을 단축시키고 비용도 절감할 수 있는 장점이 있다. 주로 판막의 협착성 병에 사용되는데 가장 많은 적응증은 폐동맥판막협착, 승모판막(이첨판막)협착, 선천성 대동맥판막협착 등이며 이외에도 혈관의 협착성 질환으로 대동맥협착증, 수술 후 발생한 폐동맥혈관분지 협착에도 사용되고 대혈관전위증 등에서 심방중격결손의 크기가 작을 때 일시적으로 혈류의 혼합을 증가시킬 목적으로 사용되기도 한다. 협착이 있다고 해서 모두 확장술의 대상이 되는 것은 아니다. 반드시 증세가 있거나 압력차가 일정한 수치를 넘어야 한다. 풍선이 심장내에서 심장을 자극하기 때문에 간혹 심장내 혈전이 있는 환자는 혈전이 떨어져 나가서 중풍을 일으키기도 하므로 시술 전에 심장 초음파 등을 시행하여 반드시 혈전의 유무를 확인하는 것이 중요하다. 확장술 후에도 확장의 정도가 불충분할 때는 수술을 해야 될 때도 있으며 또 승모판막협착에 대한 풍선확장술의 경우 너무 확장이 심하여 판막부전이 발생하면 응급수술을 해야 할 경우도 있다. 최근 판막성형술 또는 판막치환 등 심장수술의 기법이 발달되었지만 수술의 위험이 많은 환자의 경우에는 풍선확장술이 훌륭한 대치 치료법으로 자리하고 있다. 특히 여러 합병증이 겹쳐 수술을 견디기 힘든 환자, 고령의 승모판막질환자, 신생아의 심한 대동맥협착에는 풍선확장술이 효과가 좋으며 소아 폐동맥협착에서는 풍선확장술만으로 치료가 되기도 한다. |

| antral sphincter | Angular sphincter, thickening of the circular muscular layer forming a proposed intermediate sphincter at the level of the angular notch of the stomach. | balloon angioplasty | <cardiology> Use of a balloon catheter for dilatation of an occluded artery. It is used in treatment of arterial occlusive diseases, including renal artery stenosis and arterial occlusions in the leg. For the specific technique of balloon dilatation in coronary arteries, angioplasty, transluminal, percutaneous coronary is available. Coronary angioplasty is accomplished using a balloon-tipped catheter inserted through an artery in the groin or arm to enlarge a narrowing in a coronary artery. Coronary artery disease occurs when cholesterol plaque builds up (atherosclerosis) in the walls of the arteries to the heart. Angioplasty is successful in opening coronary arteries in 90% of patients. 40% of patients with successful coronary angioplasty will develop recurrent narrowing at the site of balloon inflation.
