| 설명 | 병자를 진찰, 치료하는 데에 필요한 설비를 갖추어 놓은 곳. 즉, 많은 환자를 수용하여 진단하고 치료하는 일을 하고, 또한 질병의 예방과 재활 서비스를 제공할 수 있는 시설을 갖춘 곳을 말한다. 병원은 입원환자의 의학적 진료를 위주로 하므로, 외래환자의 진료를 위주로 하는 의원이나 노인-불구자를 수용하는 시설과는 다르다. 그리고 1957년 세계보건기구(WHO)에서, 병원은 주민에게 치료와 예방을 포함하는 완전한 보건의료 서비스를 제공하는 기능을 가진 사회조직의 하나라고 정의하였다. 병원의 원형은 고대문명의 발상과 함께 시작되었고, 고대에는 신전이 병원역할을 겸하였다. 로마시대에 나환자-신체장애자-맹인-빈민환자를 위한 수용시설이 있었고 당시에 동양문명권에도 많은 의료시설이 설치되어 있었다. 중세기에는 의료가 귀족의 특권이었으며 병원은 환자를 진료하는 곳이라기 보다 고아-빈민-노인-불구자 등의 단순한 수용시설에 불과하였고, 병원은 매우 혼잡-불결하고 비위생적인 곳이었다. 문예부흥과 종교개혁 이후에 자선병원이 많이 설립되었고, 비로서 의사들이 병원을 방문하여 진료를 하게 되면서부터 의료시설의 기능을 갖추게 되었다. 이와 같은 의학의 발전이 병원으로 하여금 의학-의료의 중심지가 되게 하였다. 1920년대 미국에서 병원표준화운동이 일어나 병원의 시설-장비-의료기술수준의 향상을 가져왔고, 근대적인 간호교육과 병원관리학의 발달, 병원관리자의 양성을 통해 병원의 발전을 더욱 촉진시켰다. 한국의 경우 서양식 현재병원의 도입은 19세기 말의 일로 그 역사는 얼마 되지 않으나, 고려 때에 이미 국립기관으로서 의약-치료를 맡는 태의감이 설치되었다. |

| 설명 | 다발성 원발성골종양. 머리뼈-갈비뼈-복장뼈-척추뼈-골반 등에 잘 나타나고, 물렁물렁한 종괴를 형성하며, 뼈의 흡수가 일어나고, 40~60세 남자에게 많이 발생한다. 골수종 종양세포는 형질세포에서 유래한 것이어서 형질세포종이라고도 한다. 과거에는 형질세포성골수종 이외에는 다른 골수조혈요소에서 생기는 골수종이라고 생각했지만 현재는 부정되고 있다. 형질세포는 원래 면역글로불린을 생산하는 세포이며, 그것이 종양화한 다발 골수종 환자에서도 대부분 혈청 속에 면역 글로불린이 증가된 것을 볼 수 있다. 증가한 글로불린은 IgG나 IgA인 경우가 많지만 다른 형도 있다. 골수종 환자 약 50%는 오줌에서 벤스죤스단백질이 검출되는데, 이 단백질의 축적에 의해 요세관이 파괴되고, 콩팥경화가 일어난다. 골수종 환자에서는 혈청단백 이상으로 가끔 아밀로이드증이 나타난다. 뼈 X선 소견으로서는 도려낸 병터, 골융해상, 병적골절이 관찰된다. |

| cancer, multiple myeloma | A bone marrow cancer involving a type of white blood cell called a plasma (or myeloma) cell. The tumour cells can form a single collection (a plasmacytoma) or many tumours (multiple myeloma). Plasma cells are part of the immune system and make antibodies. Because patients have an excess of identical plasma cells, they have too much of one type of antibody. As myeloma cells increase in number, they damage and weaken the bones, causing pain and often fractures. When bones are damaged, calcium is released into the blood leading to hypercalcaemia (excess calcium in the blood) and that causes loss of appetite, nausea, thirst, fatigue, muscle weakness, restlessness, and confusion. Myeloma cells prevent the bone marrow from forming normal plasma cells and other white blood cells important to the immune system so patients may not be able to fight infections. The cancer cells can also prevent the growth of new red blood cells, causing anaemia.
