| 설명 | 심장은 심장동맥이라는 혈관으로부터 혈액을 공급받는다. 이 심장동맥은 왼심실에서 나와서 몸전체에 피를 공급해 주는 대동맥이 시작되는 부위에서 시작하여 심장을 둘러싸고 있는 형태로 존재한다. 크게 심장동맥은 3개의 분지로 나뉘는데 우선 왼심장동맥과 오른심장동맥으로 나뉘며 다시 왼심장동맥은 왼휘돌이동맥과 왼앞내림동맥으로 나누어진다. 협심증이란 심장동맥의 부분적 폐쇄에 의해서 평상시에는 증상이 없지만 심장이 많은 활동을 할 경우에 심장에 피가 충분한 만큼 공급이 되지 않아서 생기는 질병이다. 1. 임상증상 평상시에 쉬고 있는 경우에는 아무런 증상이 없지만 심장이 많은 일을 해야할 경우(운동이나 정신적 흥분, 과도한 식사, 흡연)에 증상이 생긴다. 즉 많은 일에 해당하는 만큼의 산소와 영양분을 공급받지 못하는 심장에서 증상이 생긴다는 것이다. 주증상은 가슴 부위의 통증이다. 가슴의 명치부위의 쥐어짜는 듯한 통증으로 30초에서 3분 정도의 기간을 가지며, 대개 5분이상 지속되지는 않는다. 통증은 왼쪽팔의 안쪽으로 뻗치며, 운동이나 흥분에 의해서 유발되며, 휴식이나 안정 그리고 니트로글리세린이라는 약제에 의해 완화가 되는 특징을 가지고 있다. 2. 발생원인 대개 심장동맥의 동맥경화증에 의한다. 동맥은 3개의 층으로 이루어져 있다. 가장 안쪽의 층을 혈관내막, 중간의 있는 층을 혈관 중간막, 가장 바깥쪽에 있는 층을 혈관 바깥막이라고 한다. 동맥경화증이란 동맥의 내막에 지방과 콜레스테롤로 이루어진 죽종(atheroma)가 생기는 병이다. 죽종이 생긴 동맥은 죽종이 혈관의 내부로 돌출 하게 되어서 혈관의 내경이 작아지게 된다. 이로 인해서 혈류가 감소하게 되고 계속 진행이 될 경우에는 완전히 동맥을 막을 수도 있다. 대개 식사와 혈중에 있는 지방의 농도와 관련이 있다고 한다.  |
