| 설명 | 쿠싱증후군이란 글루코코르티코이드가 만성적으로 과잉분비에 의해서 일어나는 병을 말한다. 원인으로 ① 뇌하수체에서 ACTH가 과잉 분비되는 경우: 뇌하수체에서 ACTH가 너무나 많이 분비되는 경우에 부신겉질을 자극해서 많은 양의 글루코코르티코이드가 분비된다. ② 뇌하수체 이외의 부분에서 ACTH가 많이 분비되는 경우: 폐암, 난소암 등의 종양에서 ACTH를 생산하는 경우가 있다. 이 경우에도 역시 부신피질이 자극이 되어서 많은 양의 글루코코르티코이드가 분비된다. ③ ACTH의 자극이 없이 부신에서 많은 양의 글루코코르티코이드가 나오는 경우. ACTH자극에 관계없이 부신피질의 과잉성장, 종양에 의해서 많은 양의 글루코코르티코이드가 분비되는 경우. ④ 외인성, 의인성: 치료를 위해서 많은 양의 글루코코르티코이드를 장기 투여할 경우에 생기는 쿠싱증후군 쿠싱증후군에서 특히 ①번에 해당하는 것을 쿠싱병라고 한다. 증상은 뚱뚱하고 얼굴이 달덩이처럼 둥글고 살이 쪘지만 팔, 다리는 가늘고 대신에 몸에 많은 살이 붙어있다. 목뒤에 많은 살이 있어서 그것이 덩어리를 형성하기도 한다. 이것은 원래 글루코코르티코이드가 지방을 분해하는 역할을 하지만 병적으로 많이 나올 경우에는 지방의 분포를 변화시키는 역할을 하기 때문이다. 즉 팔, 다리 등의 온몸에 퍼져있는 지방을 몸통쪽으로 모두 이동시킨다. 잔털이 온몸에 걸쳐서 많이 나있고 여드름이 많고 배에 자주색 선조가 있는 경우가 많다. 그리고 글루코코르티코이드가 단백질을 분해해서 당류를 만드는 역할을 하여 근육이나 뼈대의 심한 소실이 있다. 그래서 근육이 거의 없어지고 뼈는 아주 부러지기 쉽게 된다. 대개 고혈압인 경우가 많고 심리적으로 우울증이나 과민성 등이 있을 수도 있고 심한 경우에는 정신병증세를 보이기도 한다. |

| 설명 | 고혈압에 의해서 생기는 심장병. 고혈압심장병이라는 진단을 붙이기 위해서는 최소한 다음과 같은 조건이 부합되어야 하는데, 첫째 심장혈관계에 심장병을 유발할 수 있을 만한 다른 병변이 없이 좌심실 비대가 있어야 하며, 둘째 고혈압을 앓았다는 병력이 있어야 한다. 주로 고혈압에 의한 심장병은 초기에는 좌심실이 비후라는 것으로 특징되어진다. 즉 혈압이 높으므로 혈액을 순환시키기 위해서는 그만큼 심장의 혈액을 보내는 힘이 좋아야 한다. 그 힘을 얻기위해서는 심근의 비후가 필요로 하여 좌심실 근육의 비후가 생긴다. 그리고 고혈압이 지속이 될 경우에는 결국 심장이 제 구실을 하지 못하고 펌프로서의 기능을 잃어버리게 되어 심장기능상실에 빠지게 된다. |

| 설명 | 골반주위의 장기에 발생하는 염증을 말함. 주로 여성에서 발생하며 원인은 임균(gonococcus)과 비임균에 의한 감염(non-gonorrheal infection)에 의한다. 증상은 초기에는 질분비물, 하복부동통, 여성의 생식기부위에 발생하는 압통, 월경통, 월경량의 증가 등이다. 일찍 치료해야 하며, 계속적으로 병이 지속시 여성의 불임의 원인이 된다. 후진국에서는 가장 많은 여성불임의 원인이기도 함. 치료는 항생제의 투여이다. |

| 설명 | 갑상샘의 비대와 갑상샘호르몬의 과다분비가 특징인 병이다. 이 병은 주로 25~50세에 호발하고 주로 여자에게서 많이 생긴다. 갑상샘 호르몬은 저장되어 있는 에너지를 소모하여 신체의 대사율을 높이는 호르몬이므로 입맛이 좋은 데도 불구하고 계속적인 체중의 감소, 그리고 축적된 에너지를 소모하여 열생많이 하여서 더위를 참기 힘들어하고 만성 쇠약감이나 근력의 약화를 보일 수가 있다. 그리고 눈에 특징적인 증상이 나타나는데 눈꺼풀이 비정상적으로 위로 올라가 있고, 눈이 아래의 물건을 주시할 경우에 눈꺼풀이 정상적으로는 처져야 하지만 갑상샘 호르몬이 과도하게 나올 경우에는 눈꺼풀이 처지지 않는다. 또 눈알이 앞쪽으로 돌출하는 안구돌출을 볼 수가 있다. 또 피부가 아주 부드럽고 물기가 많아서 축축하다. 그리고 특징적으로 하지의 앞쪽에 피부가 두꺼워져 국소적 융기를 이루는 것이 있는데 이것은 이 병의 특징적인 병터이다. |

| Cushing, Harvey | <person> This leader of neurosurgery was born in Cleveland, Ohio, graduated from Harvard in 1895, remained at Massachusetts General Hospital for one year, spent 15 years at Johns Hopkins, returned to Harvard as Professor of Surgery, and retired in new Haven where he continued actively until his death. Cushing's accomplishments in neurosurgery were amazing. In 1906, he demonstrated the relationship between pituitary tumours and sexual infantilism. In 1917, he published a classic, "The Pituitary Body and Its Disorders." Cushing wrote many charming essays, some of which were published as a collection under the title, "Consecratio Medici" in 1928. He was an earnest collector of books and bequeathed his valuable collection to the Yale Library. Among his many publications were a Biography of 0sler, and a Biography of Andreas Vesalius. | Cushing's basophilism | <endocrinology, syndrome> An increased concentration of glucocorticoid hormone (ACTH) in the bloodstream that is being produced by an adrenal gland tumour (adenoma). Ectopic Cushing syndrome refers to the production of ACTH in a location other than the pituitary gland or adrenal gland. Examples of ectopic sites include thymoma, medullary carcinoma of the thyroid, pheochromocytoma, islet cell tumours of the pancreas and oat cell carcinoma of the lung. Symptoms include weight gain, central obesity, moon face, weakness, fatigue, backache, headache, increased thirst, increased urination, impotence, mental status changes and muscle atrophy.
