| 설명 | 심장의 박동이 정상적인 심장 박동이 아니라 이상하게 뛰는 것을 부정맥이고 한다. 즉 비정상적인 심장의 박동을 부정맥이라 한다. 부정맥을 이해하기 위해서는 심장의 정상 박동의 원리를 알아야 한다. 박동의 신호는 특수한 장치에 의해서 발생 되고 특수한 전도계를 통해서 전체의 심장으로 전달된다. 심장 박동의 신호는 우선 우심방에 위치하는 굴심방결절(SA node)라는 곳에서 생긴다. 여기에서 보내는 전기적 신호가 바로 심장을 뛰게하는 신호인 것이다. 이 신호로 말미암아 심방이 우선 수축한다. 그리고는 심방과 심실사이에 위치하는 방실결절이라는 곳에 일정하게 정해진 통로 없이 전체의 심방을 통해서 전달이 되고 신호를 받은 방실 결절은 히스 다발(His bundle)과 프로킨예 섬유(Purkinje's Fiber)를 통해서 심실의 심장근육에 이 신호를 보내주어 심실의 근육을 수축하게 한다. 히스발 프르킨예 섬유 전체의 심실의 심근섬유에 동시에 전기적 신호를 보내어 줄 수 있게 하는 일종의 전기줄과 같은 역할을 한다. 그리고 전기적 신호를 받아서 수축한 심장근육은 일정한 시간이 지나기 전에는 전기적 신호를 받아도 수축하지 못한다. 심근의 수측 직후에 생기는 전기적 신호를 받아도 수축하지 못하는 이런 시간을 불응기라고 한다. 만일, 굴심방결절이 심장의 박동의 신호를 만들어내지 못하는 상태가 되면 즉시 굴심방결절이 대신해서 신호를 만들게 된다. 그리고 굴심방 결절이 신호를 만들지 못하게 되면 심장의 근육들이 스스로 전기적 신호를 만들어서 심장을 뛰게 만든다. 부정맥은 이런 심장의 특성 때문에 생긴다. 즉 심장의 거의 모든 세포가 스스로 전기적 신호를 만들고 심장을 뛰게 할 수 있는 능력이 있기 때문에 부정맥이 생기는 것이다. 위와 같은 일련의 전기적인 작용 이외와 다른 부분에서 전기적 신호가 생기고 전달될 경우엔 심장의 근육이 정상적인 전기적 신호외에 다른 신호에도 호응하여 심장이 수축한다. 이렇게 되면 심장의 박동이 정상을 벗어나게 되어 부정맥이 생긴다. 그리고 이외에 부정맥의 다른 원인으로는 심장 전도계의 이상을 들 수가 있다. 굴심방결절에서 생긴 전기적 신호가 심실의 각 근육에 이르기까지 완전히 도달해야 하는데 전기를 전달하는 위에서 설명한 일련의 전기적 통로(전도계)의 부분적 흑은 완전 폐쇄의 경우엔 심장근들이 전기적 신호를 받지 못해 수축하지 못하기도 하고 또는 정상적인 박동의 형태를 갖추지 못하여 부정맥이 생길 수도 있다.  |
