    양다리에 체중을 싣고 일어나 신체를 이동시키는 보행이 전혀 불가능하거나 곤란한 상태. 성인의 경우에는 뼈, 관절의 중대한 외상이나 만성관절류머티즘을 비롯하여, 뇌혈관 장애에 의한 반신마비, 척수손상, 파킨슨 증후군, 근위축성 측색경화증 등이 원인이 된다. 어린이의 경우는 심한 정신 지체, 뇌성 마비, 진행성 근위축증, 진행성 근디스트로피 등에 의한 것이 있다. 한때는 선천성 고관절 탈구에 의한 보행장애를 볼 수 있었으나, 요즈음에는 조기진단, 조기치료로 거의 볼 수 없게 되었다. 발을 땅에 끄는 것처럼 걷는 절뚝거림은 보행이상이라 하여, 보행장애와는 별도로 다루어지지만, 그 원인과 정도에 따라서는 ‘보행곤란’으로 분류된다. 절뚝거림은 보행은 가능하지만 그 걸음걸이가 정상이 아닌 것으로, 여기에는 여러 종류가 있다. 좌골신경통 등의 아픔을 경감시키기 위하여 발을 끄는 회피파행, 선천성 관절 탈구 등에서 볼 수 있는 골반이 내려와서 걸을 때마다 환부 쪽의 어깨가 심하게 오르내리는 추락성 파행, 하지의 이완성 신경마비 때문에 대퇴부를 올릴 수 없어서 발을 끄는 마비성 파행, 뇌졸중 후의 반신마비나 척추 종양 등의 추체로 장애에 의한 하지 전체의 근긴장 때문에, 좁은 보폭으로 앞으로 쓰러질 듯이 발을 끄는 경성 파행을 비롯하여, 관절 강직이나 구루병 외에 하지의 동맥협착에 의한 간헐성 파행이나, 다발성 경화증, 진행성 마비 등에서도 일과성 파행을 볼 수 있다.
