associated with melena most commonly occurs after bleeding into the esophagus, stomach, or upper small bowel. Hematemesis, with or without melena, most often occurs from bleeding above the ligament of Treitz. Hematemesis may be bright red or darker as a result of the conversion of hemoglobin to hematin or other pigments by hydrochloric acid in the stomach. The color of vomited or aspirated blood from the stomach cannot be used to differentiate between arterial and venous sources.
