| 설명 | 피부염이라고 하면 이것은 습진피부병(eczematous dermatoses)을 이르는 말이다. 습진이란 급성기에는 가려움증을 동반하는 수포나, 홍반, 부종 등이 나타나고 만성기에는 부종, 수포는 감소되는 반면에 태선화, 비늘, 피부색의 변화 등이 형성되는 모든 피부병들을 이르는 말이다. 그 발생빈도는 지역에 따라 다소 차이가 있으나 전체 피부질환의 약 1/3~1/4을 차지하고 있다. 1.접촉피부염(contact dermatitis)-외부물질의 접촉에 의해서 발생하는 피부염을 말한다. 물이나 세제에 장기적 접촉에 의해서 손끝부위에 생기는 주부습진(housewife's eczema)이나 공장에서 유독한 물질의 접촉에 의해서 생기는 피부염 등이 이것의 예이다. 2.아토피피부염(atopic dermatitis)-피부가 가려움에 대한 역치가 낮아서 아주 심한 가려움을 느끼고 이로 인해서 2차적으로 생기는 습진으로 유전적인 경향이 아주 강하다. 아주 심한 가려움증과 특정한 발병부위로 대개 진단이 되는데 2세이하의 유아에게 생겼을 경우에는 주로 뺨, 이마, 두피 등의 얼굴에 많이 생기고, 소아기의 경우에는 주로 무릎이나 팔꿈치의 살이 접히는 부위에 많이 생긴다. 1세 미만의 유아에서 많이 발생하며, 나이가 들수록 호전되는 경과를 보인다. 3.지루 피부염(seborrheic dermatitis)-만성의 경과를 취하는 습진의 일종으로 주로 피지의 분비가 왕성한 부위(머리덮개, 얼굴, 가슴 등의 중간 부위)에 노란색 내지 붉은색의 만성 습진의 양상을 나타내는 병변, 특히 번들번들한 비늘이 특징적으로 하는 병변을 주 특징으로 하는 질환이다. 원인은 확실하지 않으나 Pityrosporum ovale이라는 균과 가족력, 신경병과 연관이 있다고 한다. |

| 설명 | 피부에 생기는 모든 병의 총칭. 피부는 신체의 표면을 덮고 있으므로 외계로부터 자극이나 여러 병원체에 직접 접촉될 기회가 많고, 체내로부터 영향을 강하게 받는다. 더욱이 피부의 근소한 변화도 눈으로 보고 손으로 만질 수 있으며, 병변부의 일부를 채취하여 병리조직학적으로 검사하거나, 미생물의 검색 등의 검사를 하기 쉽고, 개개의 질병의 진단을 명확하게 하는 것이 용이하므로, 그 종류는 다른 장기의 그것에 비하여 상당한 수에 이르며, 병명도 복잡 다양하다. 크게 나누면 다음과 같다. 변화가 피부에 한정된 것에는 반점-점 등이 있고, 전신에 미치는 것에는 아교질병, 비타민결핍성 피부병, 내분비장애성 피부병 등이 포함된다. 다만 피부를 제외한 부분을 피부에 대하여 전신으로 간주하는 경우, 변화가 피부에만 한정되는 것은 적고, 대부부분의 피부병은 혈액의 변화나 간기능장애 등의 전신현상을 수반한다. 세균-바이러스 등의 병원체가 원인인 질병에는 수두-농가진-매독 등이 있고, 각각 전염하지만, 습진-반점-종양 또는 아교질병 등은 전염하지 않는다. 피부병의 내인으로는 위장장애-간병-콩팥병-물질대사장애-혈액질환-내장종양-내분비장애 등을 들 수 있다. |

| 설명 | 1. 피부의 한 층을 이루는 조직. 피부는 크게 표피, 진피, 피하조직의 세 부분으로 되어 있다. 이중 진피는 표피를 지지하는 곳으로 혈관, 신경, 땀샘 등이 분포하고 있는 지역이다. 부위에 따라서 표피의 바로 아랫부분의 유두진피와 그 아래에 존재하는 망상진피로 나눈다. 2. 척추동물의 피부 가운데서 표피와 피하조직 사이의 부분. 혈관이나 신경 이외에 털세움근 등의 피부부속기가 분포되어 있다. 중배엽으로부터 발생하며, 두께 0.3~2.4mm의 섬유성 결합조직으로 되어 있다. 여기에는 땀샘-털주머니-기름샘 등이 있으며, 피부의 생리적 기능의 태반은 여기에서 일어난다. 표피는 베어도 출혈이 없고 아프지도 않지만, 그것이 진피까지 달하면 출혈이 있고 아픔을 느낀다. 또 진피의 얕은 층에는 흑갈색의 멜라닌 과립을 가진 세포가 산재되어 있으며, 표피의 최심층의 세포가 함유하는 멜라닌 과립과 함께 피층의 색조를 좌우한다. |
